In late 2024 the FDA approved Zepbound (tirzepatide) for moderate-to-severe obstructive sleep apnea (OSA) in adults with obesity, the first medication approved for this use. It is meant to be used along with a reduced-calorie diet and more physical activity, and it does not replace CPAP for everyone.

In the SURMOUNT-OSA program (PMID 38912654), tirzepatide reduced the number of breathing interruptions per hour in adults with obesity and OSA, alongside weight loss. Excess weight is a major driver of OSA, so losing weight can meaningfully improve it.
