Paying for Medical Weight Loss — HSA, FSA, Klarna, Affirm

What each program costs at each tier

Reading guide: the totals are the cash price; the per-month columns show what you'd pay with Klarna's 4-installment plan or Affirm's 6-month plan (rate varies by credit).

HSA / FSA eligibility, in plain language

HSA and FSA funds are eligible for "qualified medical expenses" under IRS guidelines. Compounded semaglutide and tirzepatide prescribed by a licensed physician for weight loss with documented medical necessity (BMI 30+, or 27+ with a comorbidity) qualify. The $119 physician visit qualifies. We provide itemized statements with diagnosis and procedure codes if your administrator requires them for substantiation.

What might not qualify: gym memberships (without a Letter of Medical Necessity), most fitness equipment, supplements without a prescription. Your HSA administrator confirms specifics.

Why this matters financially. If you have an HSA with a balance, every dollar paid through HSA is paid with pre-tax money — effectively a 22-37% discount depending on your tax bracket. For the $499 90-day program, that's $110-$185 in tax savings most patients leave on the table.

Klarna — 4 interest-free installments

Klarna is a buy-now-pay-later option that lets you split any purchase into 4 equal interest-free installments. The first installment is paid at checkout; the remaining three are charged every two weeks. No interest if paid on schedule. Late fees apply if you miss a payment. The signup is a soft credit check that doesn't affect your score.

For our 90-day Essential Reset at $499: 4 × $124.75 over 6 weeks. For tirzepatide at $699: 4 × $174.75. Klarna handles the financing; we receive the payment in full upfront.

Affirm — 3, 6, or 12-month plans

Affirm offers longer-term financing with monthly payments. Interest rate depends on credit profile and term length — promotional 0% APR is sometimes available; standard rates run from 10% to 30% APR. You pre-qualify online to see your rate before committing.

Why we don't bill insurance

Insurance for branded GLP-1 medications (Wegovy, Ozempic, Mounjaro, Zepbound) gets denied roughly 40% of the time. Prior authorization, when granted, takes 30 to 90 days. Many patients spend three months in back-and-forth before learning their plan rejects the medication. The cash price for branded GLP-1 without insurance is $968-$1,349/month.

Compounded therapy at our clinic is $166-$233/month. For most uninsured patients, the math favors cash-pay even before factoring in the time and stress of insurance navigation. For patients whose insurance does cover branded GLP-1 with reasonable copay, branded is the better path — we'll tell you that during the consultation.

Frequently asked questions

Do you accept HSA or FSA?

Yes. HSA and FSA cards are accepted directly at checkout for both the $119 initial visit and the medication portion of the 90-day programs. Save your receipts; we provide itemized statements on request.

Can I use Klarna or Affirm for the 90-day program?

Yes. Both Klarna (4 interest-free installments) and Affirm (3-12 month plans) are accepted for the $499 and $699 programs and the $297 Skeptic's Trial. Application is a soft credit check.

Why don't you bill insurance directly?

Insurance for GLP-1 gets denied 40% of the time and prior authorization takes 30-90 days. We chose cash-pay so patients can start within days, not months, and avoid the typical $200-$2,000 out-of-pocket gap after partial insurance approval.

What's the total monthly cost with financing?

For the 90-day Essential Reset ($499) using Klarna: 4 × $124.75 over 6 weeks. Affirm 6-month plan: ~$86/month with average APR. Compare with branded Wegovy at ~$1,349/month cash without insurance.

Are gym expenses HSA-eligible alongside this?

Generally no — gym memberships are not HSA-eligible without a Letter of Medical Necessity (LMN). We don't provide LMNs for fitness expenses; we provide them for the medication and physician services where appropriate.

Can I split payment across HSA and Klarna?

Yes. Many patients use HSA for the initial visit and Klarna for the medication. We accept multiple payment methods on a single transaction.
