Patient Story Consent Process
This page explains how to request information about a patient story and what must happen before New Hope Weight Loss considers publishing any detail.
Key point: no story is published from an inquiry alone. Publication requires a separate written authorization reviewed with the patient and signed before any story, image, or health detail is used.
Why you will not see patient stories here yet
Many weight-loss sites fill this page with quotes that cannot be checked. We would rather show you our process. A story appears here only after we verify the person's identity, confirm a real care relationship, and receive a signed written authorization covering the exact words and the exact places they will appear. No one is ever paid or rewarded for a story, and declining never affects care.
Optional and separate from care
Asking about the process is voluntary. Choosing to participate or not participate does not change eligibility, treatment, pricing, or access to the care team. A name, initials, or anonymity never substitutes for a valid authorization.
How to request information
1. Contact the team
Call (657) 837-3342 or mention the request during a scheduled check-in. Do not send medical details, photographs, or documents through a public or unsecured link.
2. Review a separate authorization
The team explains what content is proposed, where it could appear, how long the authorization applies, and which choices are available. Ask every question before deciding.
3. Decide without pressure
Nothing is published because someone made an inquiry. Only the specific content covered by a signed written authorization may be considered.
4. Understand revocation
A patient may revoke an authorization in writing, subject to its terms and prior reliance on that authorization. The team explains the procedure and limits before signature.
